Azelaic Acid vs. Niacinamide: Which Is Better for Rosacea and Sensitive Skin?

Two of the most clinically supported ingredients for sensitive and rosacea-prone skin are azelaic acid and niacinamide. Both are gentler alternatives to the typical actives that reactive skin can't tolerate β€” but they have different mechanisms, different strengths, and different ideal use cases. Here's how I think about them in my practice.

What They Have in Common

Both azelaic acid and niacinamide are anti-inflammatory, both address hyperpigmentation and uneven tone, both improve skin barrier function, and both have excellent safety profiles for sensitive and reactive skin. Neither requires a low pH to work, which means they play well with almost everything in a routine. If either of these were your only active ingredient, you'd be in good shape.

Where Azelaic Acid Pulls Ahead

For clinical rosacea β€” specifically the inflammatory subtype with papules, pustules, and significant erythema β€” azelaic acid has stronger and more direct evidence. It's used at prescription strength (15% to 20%) for this exact purpose, and at 10% it still delivers meaningful anti-inflammatory and antimicrobial benefits. If rosacea is your primary concern, azelaic acid is the more targeted choice.

Where Niacinamide Pulls Ahead

Niacinamide has broader applicability across skin types and concerns. It's particularly effective for pore minimization, sebum regulation, and as a general barrier-strengthening agent. It also has a stronger evidence base for reducing transepidermal water loss β€” meaning it keeps skin more hydrated over time. For sensitive skin without a clinical rosacea diagnosis, niacinamide is often the more versatile starting point.

Can You Use Both?

Absolutely β€” and this is actually what I recommend for many of my rosacea and sensitive skin clients. They complement rather than compete. A niacinamide-based serum in the morning and an azelaic acid treatment in the evening covers both barrier support and active rosacea management without layering irritating actives.
